For families needing ongoing support, we also offer Care Coordination Services, overseen by a Licensed Registered Nurse who brings clinical insight, compassion, and experience to every care plan. From medication management to post-hospital transitions, we work alongside families and providers to ensure seamless, quality care — and in most cases, this service is covered by Medicare.
